Assessment & Case Management
The Community Support Team at MRI works with each individual and their family to support their independence at home and in the community. This includes assessment of their work and community living skills, medical, behavioral, social, learning and personal support needs. Most important, the team will ask each individual what their personal goals are based on their hopes, desires and dreams.
Each person receiving services at MRI has a Case Manager/QIDP (Qualified Intellectual Disabilities Professional) that coordinates the person-centered plan of support. The QIDP will utilize information from the initial assessment to develop an Individual Support Plan (ISP) that will serve as a blueprint for the Community Support Team. The QIDP coordinates and supports the person with a highly qualified team of trained Direct Support Persons. Direct Support Persons assist people with their daily life skills, medical needs and money management to name just a few. All members of the team provide each person with assistance that they need while maximizing independence. The team will assist program participants with access to services to help them to lead a full and rich life in the community.
The ISP will be reviewed annually or when needed to assure continuity of service and measure progress towards the person’s goals.
