Assessment & Case Management
supporting independence at home and in the community
MRI works with each person served and their family/guardian to support their independence at home and in the community. This includes assessment of their work and community living skills, medical, behavioral, social, learning and personal support needs. Most importantly, our case management team will ask each person served what their personal goals are based on their hopes, desires, and dreams.
Each person receiving services at MRI has a Case Manager/QIDP (Qualified Intellectual Disabilities Professional) that assists with the person-centered plan of support. The QIDP will utilize information from the initial assessment to develop implementation strategies that will serve as a blueprint for the person served. The QIDP coordinates and supports the person served with a highly qualified team of trained Direct Support Professionals (DSPs). DSPs assist people with their daily life skills, medical needs, and money management to name just a few. All members of the team provide each person with the assistance that they need while maximizing independence. The team will assist program participants with access to services to help them to lead a full and rich life in the community.
The implementation strategy will be reviewed annually or when needed to assure continuity of service and measure progress towards the person’s goals.
