Community Day Services Program
Community Day Services focuses on personal, social, educational and vocational development at all ability levels. The training program includes education, community integration training, pre-vocational training, assistance with personal care and work opportunities.
Each day is planned around the individual. The individual works with his or her case manager to set goals. After goals are set by the individual, the support team works to plan an Individual Support Plan (ISP) in order to work toward the set goals.
We also offer two Community Day Services programs that promote community involvement:
- CAPS – Community Awareness, Possibilities, and Supports: offers daily learning opportunities, experiences, and activities within the community
- ECNO – Empowering Community Networking Opportunities: offers weekly opportunities to get out in the community each day by participating in various volunteer opportunities
Community Day Services are offered Monday-Friday 9:00am to 3:00pm. Transportation to and from the MRI facility is provided, if needed.
What do I need to do to be able to participate in MRI Community Day Services?
Contact Prairieland Service Coordination (PSC) in order to have a Prioritization of Urgency of Need for Services (PUNS) completed. Requests should be made by the potential client, the client’s guardian, or a primary caregiver 1 to 5 years prior to graduation. If the potential client has already graduated, requests can be made at any time. PSC can be reached at 1-800-866-8779. Private pay is also available.
What do I need to be eligible to receive Community Day Services at MRI?
- Social security number
- Medicaid Card number (if applicable)
- Phone number
- Guardianship information
- Diagnosis of a developmental disability
