Case Management & Assessment

What is case Management?

MRI works with each person served and their family/guardian to support their independence at home and in the community. This includes assessment of their work and community living skills, medical, behavioral, social, learning and personal support needs. 

Most importantly, our case management team will ask each person served what their personal goals are based on their hopes, desires, and dreams.

Each person receiving services at MRI has a Case Manager/QIDP (Qualified Intellectual Disabilities Professional) that assists with the person-centered plan of support. From there we create step-by-step plans to execute future ambitions and goals.

Direct Support Professionals

Direct Support Professionals (DSPs) play a vital role in helping people build meaningful, independent lives. They provide support with daily living skills, health and wellness needs, community engagement, and money management—always with a focus on promoting choice, dignity, and independence.

Each team member works alongside the individual to understand their goals, offer the right level of assistance, and connect them with services and opportunities that help them lead a full, rich life in the community.
